Registration Form

The DD form 2875 (AUG 2009) registration form will be utilized by all Government Employees, Government Sponsored Contractors and Foreign Nationals to register in HMIRS. Please specify either “Proprietary” or “Non Proprietary” access in Block 15, by selecting “Other”, then typing in your preference in the space provided.

Instructions for saving the DD2875:

1. Once the DD2875 is open and displayed on the screen
2. Click File
3. Click Save As
4. Save In field use the drop down menu and choose Desktop
5. File Name field, populated DD2875.pdf, request that you change this to DD2875 Last Name.pdf
6. Click Save

After you have updated the DD2875 form, we request that you follow the steps outlined below to send the form electronically:

1. Open up e-mail
2. Click to ensure that the “Digitally Sign Message” Icon is actively in use
3. Click to ensure that the “Encrypt Message Contents and Attachments” Icon is actively in use
4. E-Mail the information to DLIS Account Management, dlisacctmgmt@dla.mil

If you choose to send this information via fax, please fax it to commercial 269-961-5925 or DSN 661-5925.

*NOTE for Contractors*: To prevent any delay in the processing of your HMIRS access request, Contractors MUST type in and include the following information in Block 27 (Optional Information/Additional Information):   COR signature, COR Printed Name, Office Symbol/Code/Mail Stop, Organization, Street/PO Box, City/State/ZIP Code, City/Country (If APO or FPO address), Commercial Phone, DSN, Email Address, and FAX number.
